NEW OB Patients
Welcome to the Women’s Healthcare of Norman. We appreciate you as a prospective patient and look forward to providing you with your healthcare needs.

• Any previous OB/GYN medical records need to be faxed or mailed to our office prior to your first scheduled appointment.
• Download and complete the NEW OB Patient Packet, and mail, fax or bring in to our office.
• After the Dr. has accepted you as a patient, you will receive a phone call from our office to schedule your first OB History Appointment. Please allow 5 days for Dr. to review the Pregnancy Questionnaire.

ESTABLISHED Patients who become pregnant
Download, print and complete the NEW OB Patient Packet, and mail, fax or bring in to our office. Even though you will be able to schedule your first OB History Appointment, the Dr. will still need to review the Pregnancy Questionnaire to approve you as a OB Patient. Please allow 5 days for Dr. to review the Pregnancy Questionnaire.

You may call the office to have the Pregnancy Questionnaire mailed to you for completion.
As a courtesy, Women’s Healthcare of Norman call and verifies insurance benefits prior to your first appointment. The Benefits Coordinator will call to discuss the insurance benefits and will be available to explain the process in which the OB visits are filed to the insurance.

NON-OB New Patient
Download, print and complete the New Patient Packet, and bring it with you to your first scheduled appointment. Please be sure to bring your insurance card and picture identification. Your co-pay will be collected prior to being seen by the Dr.
